Both the Exam Application Form and the Payment Form must be submitted prior to your Application being reviewed. Please use the Checklist that is part of the download to ensure that all items are included in your submittal.

Please note that an applicant will be required to reapply for the certification examination if one of the following occurs:

· The applicant applies for the certification examination and does not take it within one year of being approved.

· The applicant applies receives a failing grade on the certification examination and does not retake the exam within one year of taking the initial examination.
